Patient and family maps

Below is the start of a fictitious example of a patient/family map, which could be developed. They are all different, rather like an expressionist painting of your feelings for a particular problem, family or patient. They are often best drawn during a discussion with a PHCT member who is involved with that particular family, or as a teaching tool.

Place the “important” members of the family in the centre of the page and link family members, relationships and problems together. Use the map to develop ideas about future management of the problem.
